The pros and cons of informed consent

◆ Pros
 Although some people are against it, I am for informed consent for the three following reasons.
 The first reason is that patients have the right to know everything about the situation and doctors have the duty to tell them. How would you feel if you had some health problem and doctors did something to your body without your consent or sufficient explanation about the situation? Nobody but you can decide what is right for you.
 The second reason is that patients have the right to refuse the treatment. After considering all the consequences, if patients decide that it is better for them to refuse the treatment, then they should have the right to do so.
 The third reason is that it will deepen people’s understanding of medicine and diseases. If all the people try to learn about medicine and diseases every time they have health problems, it will facilitate people’s better understanding of medicine, and consequently people will be able to be more thoughtful to people with physical or mental health problems, and the world will become a more considerate society.
 As I mentioned, for the aforementioned three reasons, I am for informed consent.

◆ Cons
 Although some people are for it, I am against informed consent for the three following reasons.
 The first reason is that, in many cases, when it comes to medicine, patients are amateurs while doctors are professional. It is dangerous to let patients, amateurs, have the final say. It is only reasonable to let professionals to make the final decision.
 The second reason is that there are many occasions when it is difficult to implement informed consent. For example, if someone is unconscious and the immediate treatment is required, there is no way and time for doctors to explain carefully the situation to patients. On top of that, if the patient is mentally ill, it is almost impossible for the patient to make a rational decision.
 The third reason is that some patients do not want to know about their disease. For example, some terminally ill patients express their request that they do not want doctors to tell them if the disease is incurable and they will die soon. Since mental pain of being terminally ill is excruciating and unbearable for some people, doctors should respect their request.
 As I mentioned, for the aforementioned three reasons, I am against informed consent.

◆ トピック:インフォームド・コンセントの賛否

例によって、賛成派と反対派を書きました。

賛成派では、
「患者は全てを知る権利があるし、医者は全てを伝える義務がある」
「患者は治療を拒否する権利がある」
「人々の医療や病気に関する理解が深まる」
の3つを、キーアイディアに書いてみました。

反論としては、
「末期がん患者で自分の病状を知りたくない人にまで伝えるのはどうか?」
「自殺や保険金目的など、わざと治療を拒否して、悪用する人が出てくるのでは?」
「素人がちょっと調べただけで医療を分かったような気になるのは危険では?」
などが考えられます。

反対派では、
「医学素人である患者に最終判断を任せるのは危険」
「実行するのが難しい場合も多い」
「人によっては自分の病状を知りたくない人もいる」
の3つを、キーアイディアに書いてみました。

反論としては、
「何がその人にとって正しいかはその人本人しかわからないので、やはり患者本人が最終判断をするべきでは?」
「実行が難しい場合がある事が、実行が簡単にできる場合にも、実行しない理由にはならないのでは?」
「それは一部の人で、その一部の人の為に、他の人にもインフォームド・コンセントを実行しない理由にはならないのでは?」
などが考えられますね。
